Resources & Insights

Practical guidance for senior living leaders and care teams. Explore survey preparation, corrective action, documentation, resident care, and staff education topics that can strengthen daily operations.

State Survey & Plan of Correction Support

Survey readiness starts with the systems teams use every day: clear policies, complete records, consistent staff practice, and follow-up on identified risks. OpsWell helps communities review those systems before a survey and organize a response afterward.

When deficiencies are cited, we can help review the findings, identify contributing process gaps, draft a Plan of Correction, assign responsible owners and timelines, build monitoring tools, and check that changes are sustained. Each response should match the specific citation and applicable state requirements.

Insight: What to do after a survey deficiency

A citation is a starting point for a specific correction plan. Read the statement of findings carefully and identify the rule, the observed practice, the people affected, and the evidence surveyors used. Confirm any immediate resident safety needs first.

1. Correct and look beyond the cited example

Address the identified situation, then review other residents, shifts, records, or locations where the same gap could exist. Document the actions taken and who completed them.

2. Fix the process

Ask why the gap occurred. Was a policy unclear, an order missed, a handoff incomplete, or monitoring absent? Assign an owner, teach the revised process, and make the action feasible for the team.

3. Monitor for sustained correction

Set a completion date and a repeat audit schedule. Keep evidence of training, record review, observations, and escalation when the check finds another problem. The written response must match the citation and the applicable state instructions.

For Texas assisted living, start with HHSC assisted living provider resources and the HHSC ALF health checklist. Confirm current requirements for your license and survey notice before submitting a response.
